Pay 4 Performance is here and agencies will need to secure new tools and strategies that will guarantee their success. According to a recent study on health literacy, homecare patients have the largest percentage of health illiteracy. Understanding the problem of health literacy is paramount if higher levels of self-management and patient outcomes are to be achieved. The tools for success are available to agencies now. Outcome-based patient education tools, clinical pathways and telehealth pathways and questionnaires. These tools have been proven to ensure consistency and success! This presentation reviews the findings from the 2003 National Assessment of Adult Literacy Study, the history of P4P in healthcare and the new initiative in homecare. This webinar also covers the recommended tools from accrediting bodies and CMS, provides examples of quality patient education and clinical pathways, and includes success stories from agencies who marketed their services with clinical pathways and patient education tools to increase their referrals.
