This course will prepare the clinician to use the VNA First Hospice pathway components in Horizon Hospice v10x Guideline software. It will begin with a brief history of the Hospice pathways and the integration of best practice standards. A demonstration of the day- to- day use of the pathways and CoSteps will be provided. With this standardized care approach, it can easily be individualized to meet the patient’s needs. Different scenarios will be presented. The benefits of using these Guidelines related to continuity of care and symptom management will be emphasized. Case management tools available in the system will be described and recommended use will be provided. A Clinician Field Guide and a Practice Activity handout are included with this program.
