This course will prepare the clinician to use the VNA First pathway components in Horizon Homecare v10x guideline software. A demonstration of the day-to-day use of the pathways and CoSteps will be provided. With this standardized care approach, program components can easily be individualized to meet the patients’ needs. Numerous scenarios will be presented. The benefits of using these Guidelines in Horizon Homecare v10x related to continuity and efficiency of care will be emphasized. Case management tools available in the system will be described and recommendations for use will be presented.
