With the new OASIS C process indicator on Diabetic Foot Care and Education, CMS is preparing us for the new world of Healthcare, Pay for Performance and Value Based Purchasing. In this presentation attendees will learn agency strategies from the top down to implement a Diabetes Disease Management program. Attendees will learn the "whys" of disease management and the "how's". The diabetic Self-Management Education standards will be discussed and strategies for teaching patients will be highlighted. Educational guidelines for clinicians, documentation requirements, and the essentials of patient education will be presented.
