Program Description
With the signing of national healthcare reform into law, the U.S. healthcare system faces almost immediate - and substantial - transformation. New options and delivery system models will be available for homecare as CMS moves into long-term chronic disease management. One key ingredient that is frequently overlooked or not linked to these reforms is patient education. In this presentation, attendees will learn the role of health literacy, the importance of standardized patient education and the key that quality education plays in outcomes, marketing and success in the new world of healthcare. Concrete examples of tips and tools, including the innovative,evidence-based Stoplight tools will be presented.

Meet the Presenter:
Joan Haizlip, MSN, RN, CS is an Education and Program Consultant for VNA First/Innovative Healthcare Solutions. She is responsible for education and program development on Value Based Purchasing, Disease Management, Telehealth and Clinical Pathways. Ms. Haizlip has extensive homecare experience and has held many clinical and managerial roles in home care. Ms. Haizlip has presented at local and national levels on these topics.
