CHF Disease Management and OASIS C: From Creation to Marketing...Strategies for Success


Provider approved by the California Board of Registered Nursing, Provider Number CEP15258, for 1.2 contact hours. Contact hours will be awarded to participating RNs who complete a post-test that will be available to the participant at the conclusion of the webinar.

CHF chronic disease management is a top priority in healthcare today and an important part of homecare’s business model. With the recent release of OASIS-C that includes specific indicators related to CHF, savvy homecare leaders can readily identify that reimbursement changes are looming for homecare. With P4P trials at an end, best practice strategies will soon be announced. But why wait? In this presentation, participants will learn what P4P is all about, and strategies to start a CHF Disease Management Program. Best practice tools, tips and strategies will be discussed. The role of clinical pathways, standardized patient education, staff education, and telehealth will be highlighted.

Objectives:
  1. Define the role of OASIS C and other healthcare system changes on home health disease management.
  2. Identify two (2) strategies to create disease management partnerships across the continuum with hospitals, nursing facilities and physicians.
  3. Identify the role of clinical pathways on outcomes.
  4. Identify the role of standardized patient education in a disease management program.
  5. Identify two (2) strategic tools for success in disease management.
  6. Identify one (1) “win-theme” for marketing disease management programs across the continuum.
