This course will provide an overview of the current homecare market and challenges, including P4P and best practice standards. Definitions as well as key components for disease management programs will be presented. A brief history and a description of the components of the VNA First clinical pathways and care plans will be provided. Examples of how this evidence-based standardized care approach promotes efficient care and quality outcomes will be demonstrated. The many benefits agencies have experienced using this pathway model of care will be shared.

  • To demonstrate the importance of disease management at the national level, the industry level, the agency level and the patient level.
  • To introduce VNA First clinical pathways and care plans as tools to assist the agency, the clinician and the patient in achieving cost effective disease management.

Title: #102 Nursing: Day-to-Day Use for Home Care: Demonstration and Practice Session
Recommended Pre-Requisites for this Program: 

  1. Knowledge of the Horizon Homecare v10x Guidelines, Advanced Clinical components and functionality. This course does not cover the basics of Guideline functionality or other Advanced Clinical activities of the Horizon Homecare software.

Program Description: 
This course will prepare the clinician to use the VNA First pathway components in Horizon Homecare v10x Guideline software. A demonstration of the day- to- day use of the pathways and CoSteps will be provided. With this standardized care approach, program components can easily be individualized to meet the patient’s needs. Numerous scenarios will be presented. Benefits of using these Guidelines in Horizon Homecare v10x related to continuity and efficiency of care will be emphasized. Case management tools available in the system will be described and recommendations for use will be presented. Practice sessions are integrated and encouraged throughout the session.

Program Description: 
This course allows hands-on practice session using the VNA First Guideline components in Horizon Homecare and Horizon Hospice v10x software. Emphasizing the ease of use, a demonstration of day-to- day use of these discipline-specific guidelines will be provided. Components and activities used during the Initial visit and a Routine visit will be demonstrated. These Guidelines contain episode-specific content that can easily be used to meet the patient’s need. Practice Activities are integrated throughout the session. Practice sessions are integrated and encouraged throughout the session.

Program Description: 
The goal of this session is to highlight the movement towards an evidence-based and outcome driven hospice environment. How the VNA First pathways in the McKesson Guidelines software assist agencies and clinicians to meet and exceed these new expectations will be demonstrated. The following topics will be covered during this presentation:

  • Current state of affairs of the hospice environment
  • Introduction to VNA First Pathways: background, definitions and the end-of-life model of care
  • Pathway / Guideline Components and care management tools, their purpose and intended use
  • Benefits as they relate to the clinician and the patient

Program Description: 

This course assists agencies in preparing for efficient and successful implementation of the VNA First clinical pathways and CoSteps in Horizon Home Care and Horizon Hospice v10x. This session will include a brief overview of the framework and components of the VNA First pathways and standardized care approach. How the Guidelines are ready to use “out-of-the-box” will be emphasized. A demonstration of the use of the VNA First components and recommendations for use, along with considerations and agency-specific options will be presented. Many of these considerations are outlined in the Activities Checklist document that will be provided. Other roll out and implementation tips will be provided.
